Repatriation from Sicily requires a precise set of Italian documents. Italy’s system is nationalised — the same rules apply whether the death occurs in Palermo, Catania, Syracuse, or Agrigento — but the island’s geographic isolation and the capacity of smaller comuni (municipalities) can affect how quickly each document is obtained.
The Italian documentation chain
1. Certificato di morte from the Ufficiale di Stato Civile. The Ufficiale di Stato Civile at the local comune registers the death and issues the certificato di morte. This is the foundational document. In large cities (Palermo, Catania), this takes 3 to 5 working days. In smaller comuni — Taormina, Agrigento, Ragusa — it can take 5 to 10 working days. Certified copies (copie conformi) are required; an Italian bureaucratic convention that must be respected.
2. Procura della Repubblica involvement. The Procura della Repubblica (public prosecutor’s office) investigates sudden, violent, or unexplained deaths. For Sicily, the main offices are in Palermo and Catania. An autopsia giudiziaria may be ordered before the death certificate can be issued. This adds 2 to 4 weeks to the timeline.
3. Nulla Osta Funerario Estero (ONFE/nulla osta). This is the Italian international export permit for human remains. It is issued by the Prefettura (regional government office) after the comune confirms the death registration is complete. The Prefettura di Palermo covers the western half of the island; the Prefettura di Catania covers the east. Processing time: 3 to 7 working days.
4. Embalming certificate (certificato di imbalsamazione). Required for air transport. The approved Italian funeral director prepares this with the zinc-lined coffin documentation.
5. Freedom from infection certificate (certificato di non contagiosità).
6. IATA airline cargo documentation. Prepared by the funeral director for the international leg.

Airport and cargo routing
Sicily has two main international airports:
- Palermo Falcone-Borsellino Airport (PMO): Serves west and central Sicily. British Airways operates PMO-LHR in summer; some cases route via Rome Fiumicino (FCO) or Milan Malpensa (MXP) for cargo.
- Catania-Fontanarossa Airport (CTA): Serves east Sicily including Taormina, Syracuse, and Etna. Similar UK routing via Rome or Milan if no direct cargo available.
For deaths in Trapani, Marsala, or Agrigento: Palermo is the closer hub. For deaths in Syracuse, Ragusa, or Noto: Catania.

Key local considerations
The nulla osta process in Sicily can be slower than in northern Italy. Sicilian Prefettura offices, while operating the same national rules, may have smaller administrative capacity. Some experienced repatriation coordinators advise allowing an extra 3 to 5 days compared to Milan or Rome equivalent timelines.
Deaths during August (Ferragosto, particularly around 15 August) will face near-total closure of municipal offices for at least 5 working days.
